How to Fill In a Weekly Weight Log on Paper

Weight Tracker filled digital layout on a warm background

If a weight record is useful for your purpose, choose one unit, label five evenly spaced values increasing upward, and write each measured value under its actual weekday before plotting it. Leave unrecorded days blank. A weekly graph records numbers; it does not explain changes or determine health.

Why the written value comes before the graph

A plotted point is an approximation on a small page. The exact value below it preserves the number even if the mark sits a little above or below its intended position. Keep the written record as the reference when checking the graph.

Decide whether recording weight is useful for your purpose. The printed weekdays do not require daily weighing. If you have recording instructions for care, follow those rather than choosing a frequency from the layout. If a number-focused record is unhelpful for you, this page is optional.

You can test the recording method on plain paper: date, value and unit. Add a graph only if seeing the sequence serves a clear purpose. A visual line does not explain why a measurement changed.

The current approved V24 layout has Week of, seven Sunday-to-Saturday columns, five blank scale positions and a lower Weight row. It is a weekly chart, not the earlier eight-observation period graph described in old review notes.

Area Recording job Example
Week of Identify the Sunday date 10/04
Companion note State one unit kg
Five scale positions Equal increments increasing upward 68, 69, 70, 71, 72
Weight row Preserve each exact measured value 69.8
Matching column Plot the value on your scale Sunday point

The page has no preset target weight or required direction of change. Keep units in an accompanying note; do not invent a printed Unit field. The small dot guides support plotting after the values are defined.

Fill the record in five steps

  • Choose a purpose and frequency. Use the record only if it is appropriate for you. A seven-column chart can contain fewer than seven measurements.
  • Write the week and unit. The teaching example uses Week of 10/04 and kg in a companion note. Do not mix kg and lb in the same chart.
  • Label a consistent scale. Values 68 through 72 rise by one unit between each equally spaced level. This is an illustrative scale, not a recommended body weight range.
  • Write and plot the observation. Put 69.8 under Sunday, then place a point between 69 and 70. Keep the exact written value even if the plot is approximate.
  • Review the record's accuracy. Compare points with values, preserve missing days and note a changed measurement condition when relevant. Do not assign a cause from the line alone.

A filled-in example

Weight Tracker illustrative filled example
Illustrative example; based on the approved digital layout. Physical QA pending. Tap to open the full diagram; swipe horizontally on a small screen when needed.

Illustrative example: Sunday 69.8, Monday 70.1, Tuesday 69.9 and Thursday 70.2 kg. Wednesday is unrecorded. The first three points connect, while the missing day interrupts the line before Thursday. These synthetic values demonstrate plotting; they do not recommend daily weighing or describe a customer's body.

With a one-unit interval, 69.8 lies eight-tenths of the way from 69 to 70. The sequence moves both up and down, with no target arrow or judgment. It cannot establish fat change, a health outcome or a cause.

If an actual observation falls outside your chosen range, write the exact value below and omit a misleading boundary point. Start a clearly relabeled chart if you need a different range. Do not silently change the scale after earlier points were drawn.

Three recording situations

A clinician-requested record: use the required frequency and conditions, and keep any additional requested information separately. This insert is not a validated clinical form.

An occasional personal reference: record only the actual day of a measurement. Other weekdays may remain blank; the page does not require a streak.

A measurement-condition note: write a short reference elsewhere if scales or circumstances changed. A graph without that context may suggest a comparison it cannot support.

Do not compare unlike units or interpret an unobserved day as a zero. Connecting points is optional and adds no measurement between them.

Authoritative guidance and limits

NHS England's recording guidance describes consistent conditions, the same scales and a firm, level surface. It discusses regular weekly-to-monthly intervals in that programme's context. The presence of seven day columns on this paper is not an alternative clinical recommendation.

The World of Printables creator guide offers chart formats, but its weight-loss framing is not adopted here. JOYJOY's page has no product evidence showing weight loss or another health effect. This article teaches recording and plotting only.

Design notes: a neutral weekly chart

The approved V24 design pairs a pale sage-green, dot-guided graph with exact weekday values below. Five self-labeled levels preserve the user's choice of a consistent scale, while Sunday–Saturday headings keep one printed side at a weekly scope. The reverse rotates for the landscape page-turn direction and begins another independent week. Earlier eight-record review text and V23 printing files are not treated as the current layout. These facts follow the approved master and current listing, without claiming physical paper performance or medical validation.

Common mistakes and a final graph check

  • Unequal scale increments: use the same numeric step between levels.
  • Mixing units: label one unit for the whole chart.
  • Plotting a missing day: leave it unobserved.
  • Inferring causes from a short line: retain context and avoid health conclusions.

Before turning the page, check each point against the written value. Keep uncertainty visible, and use the record only for the purpose it was made to serve.

Choose A7 or M5 for this page

Choose the size that matches your existing binder: A7 / Pocket is 80 × 120 mm with six holes; M5 / Mini 5 is 67 × 105 mm with five holes. Landscape pages read 120 × 80 mm for A7 and 105 × 67 mm for M5; these are the same finished page sizes turned sideways. The reverse artwork is rotated for the intended flip-up reading direction. A7 gives you a larger surface for handwriting. M5 makes the page smaller; it does not make every field larger or fit an A7 binder.

Compare the finished dimensions and complete hole pattern with a refill that already fits. Current listing specifications use 19 mm consecutive hole spacing; a matching number of holes alone is not proof of compatibility across brands.

Current listing prices checked October 6, 2026: A7 / Pocket $6.99 USD; M5 / Mini 5 $5.99 USD. The pack contains 15 double-sided sheets on 120 gsm paper. Binder, pens, stickers and other accessories are separate. Each side provides another page of this layout.

Allow 3–5 business days for processing where stated on the listing; processing is separate from transit. Digital product previews and public availability do not establish physical print, writing or binder-fit QA, or on-hand inventory. Check the current product page before ordering.

FAQ

Must I weigh every day?

No. The page provides weekdays, not a requirement to record daily. Use a frequency appropriate to your purpose or care instructions and leave other days blank.

Can a line tell me why weight changed?

No. The graph alone does not establish causes, body composition, a diagnosis or a health outcome.

What if a value is outside the chosen scale?

Keep the exact written value, omit a false boundary point and use a newly labeled chart if needed.
